The HerzConnect® telehealth remote care mangement program is designed to provide better patient care in the patient's home setting while involving all stakeholders for continuous medical monitoring. Together with the patient as self-manager, all involved physicians, cardiologists and the telemedical center provide a 24/7 expert medical care. By significantly reducing re-hospitalization and inpatient time, the program can improve patients' quality of life while reducing costs.
