Cardiovascular diseases, like heart failure, affect millions of people worldwide and are one of the leading causes of death. In addition, patients with heart failure often have to endure avoidable hospital stays. To meet this challenge, the Heart and Diabetes Center North Rhine-Westphalia, Germany and Siemens Healthineers have developed the telehealth program HerzConnect. Through the use of telehealth, patients diagnosed with heart failure can manage their lives independently while being connected to the care providers at all times.
