Vor kurzem wurden die ersten Patientinnen und Patienten in die FAST-III-Studie am Medizinischen Zentrum der Erasmus Universität in den Niederlanden aufgenommen. Damit startet offiziell eine multizentrische, randomisiert kontrollierte, offene Studie, in der das Therapieergebnis bei koronarer Herzkrankheit untersucht wird, nachdem die fraktionelle Flussreserve (FFR) mit jeweils unterschiedlichen Methoden ermittelt worden war. Die FAST-III-Studie wird von Professor Joost Daemen, Kardiologe am Thoraxzentrum des Medizinischen Zentrum der Erasmus Universität, Rotterdam geleitet.
Die FFR bewertet die hämodynamische Relevanz von Engstellen (Stenosen) der Herzkranzgefäße als Verhältnis des maximalen Blutflusses im verengten Gefäß im Vergleich zum theoretisch maximal erreichbaren Blutflusses desselben Gefäßes ohne Stenose. Jüngste Studien haben gezeigt, dass Läsionen, bei denen der Blutfluss um weniger als 20 Prozent reduziert ist, nicht behandelt werden sollten.
Beim Standardverfahren zur Bestimmung der FFR wird ein Führungsdraht in die Koronararterie eingeführt, der an der Spitze mit einem elektronischen Drucksensor versehen ist, um die Drücke vor und nach der Stenose zu messen. Dabei muss den Erkrankten in der Regel eine Infusion mit Adenosin verabreicht werden, um eine Belastungssituation zu simulieren. Das kann zu unangenehmen Zuständen, wie innerliche Unruhe, Luftnot oder Brustschmerz, führen.
Die FAST-III-Studie vergleicht für perkutane Koronarinterventionen die Wirksamkeit des Einsatzes von „CAAS vFFR“ (Cardiovascular Angiographic Analysis Systems for vessel Fractional Flow Reserve), einer Software von Pie Medical Imaging, mit dem Therapieergebnis nach Messung der Blutversorgung mit der herkömmlichen, invasiven, druckdraht-basierten Methode. FAST III steht für “Fractional Flow Reserve or 3D-Quantitative-Coronary-Angiography Based Vessel-FFR Guided Revascularization”.
CAAS vFFR ist eine innovative Technologie, die die nicht-invasive physiologische Bewertung von koronaren Läsionen ermöglicht. Mit ihr lassen sich Druckabfall und der vFFR (vessel Fractional Flow Reserve)-Wert anhand von Angiographie-Bildern ermitteln. Der Einsatz von invasiven Druckdrähten oder von hyperämie-induzierenden Mitteln lässt sich damit vermeiden.
In den Studien FAST I1, FAST EXTEND2 und FAST II3 wurde gezeigt, dass CAAS vFFR ein einfach zu bedienendes Werkzeug zur Beurteilung der koronaren Physiologie ist und eine hohe diagnostische Genauigkeit zur Einschätzung der Schwere einer Läsion liefert.
"In den retrospektiven Fast-I- und FAST-EXTEND-Studien haben wir kürzlich die vFFR-Software validiert und gezeigt, dass vFFR eine hohe lineare Korrelation zu invasiver druckdraht-basierter FFR und eine hohe diagnostische Genauigkeit sowie eine sehr geringe Interobserver-Variabilität aufweist. Diese Ergebnisse wurden dann in der prospektiven multizentrischen FAST-II-Studie mit Hilfe eines unabhängigen verblindeten zentralen Kernlabors bestätigt", erklärte Dr. Joost Daemen.
In FAST III wird untersucht, ob sich das Therapieergebnis für die Studienteilnehmenden mit intermediärer Koronarstenose unterscheidet, je nachdem, ob die Entscheidung für eine koronare Revaskularisation (Wiederherstellung der Durchblutung) auf FFR-Bestimmung mit CAAS vFFR oder mit Druckdraht basiert.
"Wir glauben, dass die FAST-III-Studie ein wichtiger nächster Schritt sein wird, um den klinischen Wert von vFFR bei der Bewertung der physiologischen Bedeutung von intermediären Läsionen der Herzkranzgefäße bei Patienten mit stabiler oder instabiler koronarer Herzkrankheit zu demonstrieren. Die Nichtunterlegenheit von vFFR im Vergleich zu einer FFR-geführten Behandlungsstrategie könnte ein weiterer wichtiger Schritt zur Vereinfachung der physiologischen Läsionsbewertung sein, da dadurch die Notwendigkeit entfallen könnte, Druckdraht oder Mikrokatheter zu verwenden und auf die Gabe eines hyperämischen Mittels mit bekannten Nebenwirkungen für die Patienten verzichtet werden kann", kommentierte Dr. Joost Daemen.
In die Studie sollen insgesamt 2228 Patientinnen und Patienten in sieben europäischen Ländern und an etwa 35 Krankenhäusern aufgenommen werden. Leitende klinischen Zentren werden unter anderem sein: Medizinischen Zentrum der Erasmus Universität Rotterdam (Niederlande), John Radcliffe-Klinikum Oxford (Großbritannien), Mater Private-Klinikum Dublin (Irland), Universitätsklinikum von Verona (Italien), Universitätsklinikum Valladolid (Spanien), Charité Berlin und Universitätsklinikum Lille (Frankreich).
Das European Cardiovascular Research Institute (ECRI), in Rotterdam in den Niederlanden ist Sponsor der wissenschaftsinitiierten FAST-III-Studie. FAST III wird von Pie Medical Imaging und Siemens Healthineers finanziert, die zusammenarbeiten, um die klinische Evidenz für die Verwendung von angiografie-basierter FFR weiter zu stärken.
