Siemens Healthineers hat nexaris Therapy Suites für intraoperative multimodale Bildgebung im Bereich der Chirurgie und interventionellen Radiologie entwickelt. Diese unterstützen eine präzise Diagnostik, intraoperative Echtzeit-Bildgebung und direkte Ergebniskontrolle. Klinische Arbeitsabläufe können effizienter und für den Patienten komfortabler und sicherer gestaltet werden. Auf dem diesjährigen Kongress der Radiologischen Gesellschaft Nordamerikas (RSNA) in Chicago präsentiert Siemens Healthineers nexaris Therapy Suites weltweit zum ersten Mal.
Angiographie- und CT-Aufnahmen kombinieren mittels nexaris Angio-CT
Nexaris Angio-CT ermöglicht es, Bildinformationen sehr leicht zu überlagern, denn dank "Instant Fusion"2 basieren die Angiographie- und CT-Aufnahmen erstmals auf einem gemeinsamen Koordinatensystem. "Die gemeinsame, wechselseitige Verwendung eines Angiographie-Systems und eines Computertomographen erleichtert bei kombinierten Prozeduren, wie Tumorablation und Tumorembolisation, eine schnellere und präzisere Behandlung. Schneller wird dies vor allem durch die Überlagerung von CT-Bildern mit Live-Fluoroskopie, der sogenannten "Instant Fusion'. Dank des gemeinsamen Koordinatensystems sind zeitaufwändige manuelle Überlagerungen nicht mehr notwendig. Da für die Bildregistrierung keine weitere Bildgebung mehr erforderlich ist, wird diese wertvolle Information erstmals ohne zusätzliche Strahlenbelastung verfügbar", so Dr. Michael Scheuering, Leiter der Business Line Interventional Radiology bei Siemens Healthineers. Der Patient muss nur einmal registriert werden, damit der behandelnde Radiologe die Patientendaten auf beiden Systemen abrufen kann. Beide Modalitäten können von nur einem Platz im Kontrollraum gesteuert werden.
Zwei-Raum-Lösung für optimale Systemauslastung
Somatom CT Sliding Gantry Systeme stehen auf bis zu 12 Meter langen Schienen, durch die sie je nach Bedarf an den Patienten herangefahren oder aber dem deckengehängten Artis Angiographie-System Platz machen können. Sie können zudem in einen abtrennbaren Nebenraum gefahren werden, in dem diagnostische oder interventionelle Verfahren am CT stattfinden können, während das Angiographie-System zeitgleich mit Routine-Interventionen belegt ist, für die es keines Angio-CTs bedarf. Mit dieser Zweiraumlösung wird die Systemauslastung optimiert. Nexaris Angio-CT kann bei Bedarf zudem um das kabellose Ultraschallsystem Acuson Freestyle erweitert werden, um beispielsweise den arteriellen Gefäßzugang zu erleichtern. Die Ultraschallaufnahmen können dann direkt am Bildschirm neben den Aufnahmen des Angio-CTs dargestellt und verglichen werden.
Nexaris Angio-CT besonders für Behandlungen in der interventionellen Radiologie
Nexaris Angio-CT vereinfacht Eingriffe innerhalb der interventionellen Onkologie, wie zum Beispiel das kombinierte Ablatieren und Embolisieren eines Tumors in der Leber. Im Standardverfahren muss dabei der Patient zwischen dem Angiographie-System und dem CT in separaten Behandlungen hin und her wechseln, während bei Nexaris Angio-CT alle Behandlungsschritte, von der Pfadplanung über die Embolisation des Tumors mittels Angiographie bis zur Ablation, also dem Veröden des Tumors, unter CT-Bildgebung in einer Suite ohne Patiententransfer stattfinden. Auch die Behandlung von Poly-Trauma-Patienten wird mit Nexaris Angio-CT erleichtert. Denn sobald durch eine CT-Aufnahme eine interne Blutung erkannt wurde, kann dieses direkt am Angiographie-System durch eine Katheterbehandlung geschlossen werden. "Im Fall von schweren Aufprallverletzungen können wir umgehend innere Blutungen mit CT-Bildgebung diagnostizieren um diese dann schnell und gezielt unter Zuhilfenahme des Angiographie-Systems zu stoppen; und somit potentiell das Leben des Patienten zu retten", so Dr. Mark Wilson, Chief of Interventional Radiology, Zuckerberg San Franciso General Hospital.
Lösung für intraoperative Bildgebung mit MR, CT und Angiographie in der Chirurgie
Im Bereich der chirurgischen Verfahren können mit nexaris Angio-MR-CT das robotergestützte Angiographie-System Artis pheno, die MR-Systeme Magnetom Aera oder Skyra, sowie Somatom CT Sliding Gantry Systeme als Mehrraumlösung kombiniert werden. Um auch hier eine umständliche und risikoreiche Umlagerung des Patienten zwischen den einzelnen Modalitäten zu vermeiden, hat Siemens Healthineers zusammen mit dem strategischen Partner Getinge die Transferlösung Pilot entwickelt. Wird der Patient beispielsweise am Angiographie-System behandelt, liegt er dabei auf dem Maquet Magnus-Tisch, der eine optimale Bildgebung und Positionierung des Patienten ermöglicht. Wird nun MR-Bildgebung während der Prozedur benötigt, wird der Maquet Magnus Operationstisch mit dem MR-kompatiblen Combi Dockable Table mittels Docking-Adapter verbunden. Unter der Liegefläche des Maquet Tisches befindet sich das Transferboard, bestehend aus Kevlar, einem für MR- und Röntgenbildgebung nutzbaren Material. Mithilfe dieser Unterlage kann der Patient auf den MR-Tisch geschoben werden, ohne dass seine Behandlungsposition verändert wird. Anschließend kann der Patient in den MR-Raum gefahren und dort die Bildgebung am Magnetom Aera, einem 1,5 Tesla System oder am Magnetom Skyra, einem 3,0 Tesla System für MR-Aufnahmen, vorgenommen werden.
Sliding Gantry CT für CT-Aufnahmen im Hybrid-OP
Nexaris Angio-MR-CT integriert für chirurgische Eingriffe neben MR- und Angiographiebildgebung auch Bildgebung am CT-System, das identisch zum Setup der Zweiraum-Lösung für die interventionelle Radiologie auf Schienen montiert ist. Da der Maquet Tisch des Artis pheno ebenfalls für CT-Untersuchungen nutzbar ist, entfällt hier jegliche Umlagerung. Sobald nötig, kann Artis pheno vom Tisch weggefahren und die Sliding Gantry des CTs an den Tisch herangefahren werden, um CT-Aufnahmen intraoperativ vorzunehmen.
"Diese multimodalen Bildgebungs-Suiten werden die Forschungsstätten der Zukunft sein", so Prof. Lee Swanström von dem Institut de Chirurgie guidée par l'Image (IHU) Straßburg. Zur optimalen Systemauslastung können alle genannten Systeme auch hier einzeln genutzt werden: MR und CT für diagnostische Untersuchungen und Artis pheno im Hybrid-OP für rein Angiographie-gesteuerte Eingriffe.
Erstmals intraoperative Ganzkörper-MR-Aufnahmen dank nexaris Angio-MR-CT
Nexaris Angio-MR-CT bietet Vorteile bei einer kompletten Tumorresektion, beispielsweise in der Neurochirurgie. Mittels intraoperativer Bildgebung kann der Chirurg sofort erkennen, ob er Tumorgewebe übersehen hat oder er seinen Behandlungsbereich verändern muss. Mit nexaris Angio-MR-CT wird es erstmals möglich, während des Eingriffes jederzeit MR-Aufnahmen des gesamten Körpers ohne Umlagerung vorzunehmen. Dadurch profitieren nicht nur Neurochirurgen, sondern z.B. auch Viszeralchirurgen bei Resektionen und Ablationen im Bauchraum von der dank MR-Bildgebung zusätzlich verfügbaren Darstellung des Gewebes, die potentiell einen entscheidenden Einfluss auf den Therapieverlauf haben kann.
Instant Fusion befindet sich derzeit im FDA-Zulassungsstatus "510k pending".
